1. What type of tattoo, freehand or flash (predrawn), you are interested in getting

2. Approximately how large you would like your tattoo in inches

3. Where on your body you would like the tattoo

4. If you would like a color or black and grey tattoo

5. Any special needs, requests, allergies to food, chemicals or latex that I should know about
